Please bring the following documentation to be turned in with the application:
-Proof of Residency
-Birth Certificate
-Physical
-Social Security Card
-Shot Record (shots must be up to date)
-Proof of Income (1 month's check stubs, W2, or Tax Return)
*Kindergarten*
